New COVID-19 Vaccine Requirement: As of April 2021, Loyola University Chicago is requiring all Loyola students enrolled in classes for the fall to receive the COVID-19 vaccine. The Wellness Center's email address for immunization submissions is If you submit your records via email, they MUST be in PDF format. Step 2: Submit a copy of your official immunization record (from your high school or a healthcare provider) to the Wellness Center via email, fax, or you can mail or drop them off at the Wellness Center. This also allows you to print copies of your immunization records. Step 1: Enter your immunizations on LOCUS. 2-Step Document Submission (for all vaccines except COVID) Several students this fall have incomplete immunization records and are blocked from registering for classes until they upload their immunizations. Note for students registering for classes in November: Please check LOCUS to make sure your immunizations records are complete. We ask that you please check your LOCUS account periodically before calling to check on the status of your immunization records. NOTE: Due to remote working conditions, it is taking longer to verify immunizations than previous years. *Please note, immunization requirements must be submitted and sent to pdf format even if non-compliance fee was applied, in order to register for class or make a change to your registration. Students living in University-owned housing will not be permitted to move in until the two-step process is complete. The Non-compliance fee is non-refundable, and will not be removed even after immunization information is completed. All students must enter immunization requirements into LOCUS and submit supporting documents to the Wellness Center before the 10 th day of the term.įailure to provide timely information may result in a $50 Non-compliance fee, that may be applied to your account. Incomplete immunization information will block access to registering or changing classes. All Loyola students enrolled in seven or more credit hours must complete the two-step process to verify proof of immunity. Proof of immunity is an Illinois state requirement.
